Essential Functions Statement(s)
- Responsible for
the intake of clients and setting-up of client files including all
required documentation.† Ensure
HIPAA compliant confidential client files are properly maintained.
- Coordinate and
lead IP meetings; develop plans approved in IP meetings; train Direct
Support Workers in implementing IP plans and documentation requirements.
Develops, maintain and manage associated costs budgets as required by
monitoring and supervision of the delivery of services as required.† Document and maintain changes in
authorized client services.
- Prepare reports
and ensures Continuous Quality Improvement (CQI) as required by funders.
- Assist with staff scheduling efforts as needed to
ensure proper staffing levels are maintained.
- Substitute for other Service Supervisors and Direct Support
Workers, as needed.
- Review and approve Billing, Payroll, Mileage.
- Assist with training and development of Direct Service Workers for
assigned client in accordance with the policies and procedures of the

Support Services Group
- Assist Manager with public awareness; provide interested
individuals with information about services and/or facilities available;
perform other services pertinent to client services, such as attending ISP
and IEP meetings.

Bachelorís degree from an accredited
college or university AND
Experience: †One (1) years of verifiable experience working
directly with people with disabilities performing verifiable, relevant work
Skills:† Effective computer skills with proficiency in the use
of Word, Excel, e-mail and an online data management system.
Other Requirements:† Must be able to
maintain an acceptable criminal abstract from the State of Hawaii, as
determined by the company. Must be able to maintain
Protective Services Central Registry clearances from Adult Protective Services
(APS) Central Registry and Welfare Services (CWS) Central Registry. Must maintain CPR and First Aid Certificates.† Must pass a pre-employment physical, drug
screening and maintain annually a current TB clearance. Must
have a current/valid driverís license and acceptable traffic abstract.
